Schweitzer, CNP, Horizon Health Care Provider of the Year


 

 

HOWARD, SD – Horizon Health Care recently recognized Carla Schweitzer, CNP, as the Horizon Provider of the Year. This award is designed to recognize one medical or dental provider each year who embodies the spirit of care Horizon strives to provide its patients and communities. Selection is based on a number of items, including patient satisfaction, quality of care, service to community, and effective leadership. This is the third year a provider has been recognized with this honor.

Carla has provided medical care in Eagle Butte for the past 14 years. She joined the organization through the Horizon Health Care/Prairie Community Health Center merger in 2015, but had been with Prairie Community Health for ten years prior to the merger.

Carla’s investment in patient care is evident through her patient satisfaction scores. 95% of her patients indicated that she is the “best provider possible”. Carla maintains excellent rates of hypertension control and cervical cancer screening, and provides diabetic education to patients in her clinic and community. One of the qualities Carla’s team admires most about her is the passion she displays towards her pediatric patients. Outside of providing care at the clinic in Eagle Butte, Carla also travels to Tiospaye Topa to deliver high-quality care to Horizon’s only school-based clinic.

Carla is a compassionate leader within her care team, co-workers, and in her community. She is also quick to credit her care team for their achievements. “I could not do this without my team at the clinic. No one can stand out in a team without all of the individuals being awesome and doing their part.”

Carla resides near Glencross, SD, where they farm and ranch. She is the mother to two boys and grandmother to five grandchildren and one step-granddaughter.
